includes and excludes

Includes and excludes will be sets of (optionally qualified) names.

Pages named in the excludes will be used as visible pages (topics) when resolving references.

Pages named in excludes will be excluded from topics and will not be used when resolving references.

Includes are "inherited" by sub-pages, but can be effected by "closer" includes and excludes.
