Thinking about the journal display

I've tracked down a few bugs and extended the display for the Journal.

• Topic is now the first item listed.
• Command and Operation names are now both displayed
• Changes in the relationships to other well known items are also displayed.

Unfortunately, the journal display is currently restricted to transactions applied to the current page/file. That's two narrow. For example, in the relations1 example, the well known has an associated dictionary. And its transactions are not being displayed.

Further, when a page is added to a data set, the transaction currently only displays in the journal for the dataset.

So I think the transactions included in the journal display need to be broadened.
